2018糖尿病足患者血浆同型半胱氨酸水平的变化及其临床意义

  [摘要] 目的 研究糖尿病足患者血浆同型半胱氨酸水平的变化及其临床意义。 方法 选取2014年4月―2015年5月该院收治的75例糖尿病患者为研究对象,分为糖尿病足组(DF组)37例和糖尿病组(DM组)38例,以及31名健康者作为对照组(NC组)。检测Hcy、FFA、C肽及其它生化指标,分析各组之间Hcy水平的变化以及多种生化指标与Hcy升高的相关性。 结果 DF组Hcy水平显著高于DM组和NC组,均差异有统计学意义(P http://

  Changes and Clinical Significance of Plasma Homocysteine Level of Diabetes Foot Patients

  [Abstract] Objective To research the changes and clinical significance of Plasma homocysteine level of diabetes foot patients. Methods 75 cases of diabetes patients from April 2014 to May 2015 were selected as the research objects and divided into the diabetic foot group with 37 cases and diabetes group with 38 cases and 31 cases of health people were selected as the control group, and the Hcy, FFA, C peptide and other biochemical indexes were tested and the changes of Hcy level and correlation between most biochemical indexes and increase of Hcy in each group were analyzed. Results The Hcy level in the DF group was obviously higher than that in the DM group and NC group(P    DF组的相关因素分析Hcy与年龄、病程、HbA1c、FFA呈正相关;与HDL-C 、C肽呈负相关。二元Logistic回归分析显示Hcy是糖尿病足的影响因素之一。这或是因为患者随着年龄的增长、病程的延长,胰岛β细胞的储备和分泌功能逐年下降,C肽逐年下降,血糖控制越来越困难,糖化血红蛋白逐年上升,胰岛素缺乏,胰岛素抵抗越来越明显,影响Hcy的分解代谢,而产生高Hcy血症,患者长期血糖控制不佳,血流处于高凝状态,有利于血栓形成,FFA升高,以致出现脂质代谢紊乱,保护性的HDL-C下降,促进斑块形成,血糖长期升高还导致糖基化终末产物增多,血管内皮细胞受损,加速动脉粥样硬化[11],最终导致糖尿病足的发生。
  综上所述,该研究显示,Hcy是糖尿病足的影响因素之一。DF组患者Hcy水平显著升高与年龄、病程、HbA1c、HDL-C,FFA,C肽与其有相关性,研究提示可以将Hcy作为其DF的预警和诊断的实验室参考依据之一。该研究为横断面研究,样本量小,未能得出Hcy与糖尿病足的因果关系,若能进一步扩大样本量进行前瞻性的研究,得出的结论将更可靠。DF组患者Hcy水平显著增高的确切机制尚不清楚,还有待于进一步深入探讨。
