2018最小ADC值对侧脑室中枢神经细胞瘤诊断和鉴别诊断的价值

  [摘要] 目的 通过最小ADC值分析,以提高对侧脑室中枢神经细胞瘤(CNC)诊断及鉴别诊断的准确性。 方法 回顾性分析江苏省人民医院2010年9月―2016年9月间经手术病理证实的41例侧脑室肿瘤的常规MRI、DWI表现,分成CNC组、室管膜瘤组,星形细胞瘤组,并对其最小ADC值进行统计分析。 结果 18例CNC DWI表现为高信号12例,稍高信号6例,平均最小ADC值为(0.65±0.13)×10-3 mm2/s;23例非CNC DWI为等或稍信号16例,低信号7例,其中12例室管膜瘤平均最小ADC值为(1.07±0.15)×10-3 mm2/s,11例星形细胞瘤平均最小ADC值为(1.14±0.19)×10-3 mm2/s;CNC组平均最小ADC值与其他两组差异有统计学意义;根据ROC曲线,当肿瘤最小ADC值为0.85×10-3 mm2/s,诊断CNC敏感性为94%,特异性为100%,阳性预测值为94%,阴性预测值为96%。结论 最小ADC值对于侧脑室CNC的术前诊断及鉴别诊断有明确的应用价值。

  Value of Minmum ADC Value in Diagnosis and Differential Diagnosis of Paracoele Axoneuron Tumors

  [Abstract] Objective To analyze the minimum ADC value in order to improve the diagnosis and differential diagnosis accuracy of CNC. Methods The routien MRI and DWI manifestations of 41 cases of patients with tumors of lateral cerebral ventricle proved by the operation from September 2010 to September 2016 were reviewed and divided into the CNC group, ependymoma group and astroma group, and the minimum ADC value was statistically analayzed. Results Of 18 cases, there were 12 cases whose CNC DWI manifestations were high signal and 6 cases with slightly high signal, and the average minimum ADC value was(0.65±0.13)×10-3 mm2/s, of 23 cases, there were 16 cases whose non- CNC DWI equal or slightly low signal, and 7 cases with low signal, and the average minimun ADC value of 12 cases of patients with ependymoma was (1.07±0.15)×10-3 mm2/s, the average mean ADC value of 11 cases of astrocytoma patients was(1.14±0.19)×10-3 mm2/s, and the difference in the average minimum ADC value between the CNC group and the other groups was statistically significant, according to the ROC carve, when the minimum ADC value of tumors was 0.85×10-3mm2/s, the diagnosis CNC sensitivity, specificity, positve predictive value and negative predictive value were respectivley 94%, 100%, 94% and 96%. Conclusion The application value of minimum ADC value in prooperative diagnosis and differential diagnsois is clear.

  中?猩窬?细胞瘤(Central neurocytoma,CNC)是一种少见的侧脑室肿瘤,2007年WHO分类正式将CNC归为神经元和混合型神经元胶质细胞肿瘤范畴,分级为Ⅱ级[1]。目前对于CNC的诊断大多根据常规MRI表现,但侧脑室区肿瘤种类较多,因此给诊断与鉴别诊断造成一定的困扰。有文献报道DWI对CNC诊断所帮助[2];因CNN好发年龄为20~40岁的青年人[3],而成人侧脑室区非CNC且同为WHOⅡ级以星形细胞瘤[4]与室管膜瘤发病率较高,所以该研究利用最小ADC值对2010年9月―2016年9月间收集的41例侧脑室内CNC与室管膜瘤和星形细胞瘤进行鉴别,采用接受者工作特征(ROC)曲线确定诊断CNC的最小ADC阈值,期望为术前明确诊断CNC提供有力的帮助,现报道如下。     1 资料与方法
  1.1 一般资料
  回顾性分析江苏省人民医院经手术及病理证实的18例侧脑室内CNC的临床及影像学资料,分为CNC组,其中男10例,女8例,年龄10~64岁,平均(34.8±14.9)岁。同时收集侧脑室内非CNC肿瘤 23例,包括室管膜瘤组12例,男7例,女5例,年龄20~65岁,平均年龄(45±19.8)岁;星形细胞瘤组(WHOⅡ级)11例,男7例,女4例;年龄19~55岁,平均(38.2±16.1)岁。3组资料主要临床症状多为头痛、头晕、恶心、呕吐、行走不稳等。
  1.2 方法
  1.2.1 检查方法 采用SIEMENS公司3.0T Trio磁共振成像扫描仪,行横轴位T1WI,T2WI,FLAIR,弥散加权成像(DWI)及矢状位T1WI后,予静脉注射对比剂Gd-DTPA(用量为0.1 mmol/kg)行 T1WI 横轴位、冠状位和矢状位增强扫描。扫描层厚均为5 mm,层间距1.25 mm;矩阵128×128,FOV 220 mm×220 mm。
  1.2.2 图像分析与测量方法 分析所有病例常规MRI表现、DWI及ADC图信号强度。取ADC图为低信号且增强后强化最为明显的肿瘤实性区域设置感兴趣区(ROI),避开囊变、出血及血管,ROI>5 mm2,测量3次,取其最小值。
  1.3 统计方法
  采用SPSS 18.0统计学软件分析数据,计量资料以均数±标准差(x±s)表示,以P    ?C上所述,常规MRI表现与DWI对术前诊断侧脑室CNC有一定价值,而最小ADC值测量为鉴别诊断CNC提供一种简单可行的方法。
